Performing Band - Nashville, Tennessee - April 23-26, 2020

Trip payments can be made on the web site. You must click on Log In or Sign Up in the top right corner of the web page. You will have to sign up first. Then, you will be able to log in to make payments by credit card or PayPal. This way you can keep track of your payments.

Travel Package:

  • All taxes. Meal, tour leader, and motorcoach driver gratuities, where applicable..

  • Complete insurance including $5000 Emergency Accident, $1000 Life, $250 Emergency Dental

  • 3 nights lodging with night-time supervision at Holiday Inn Express Nashville SE Antioch

  • Admission to The Parthenon, Nashville Symphony, Festivals of Music, Ryman Auditorium, Hall of Fame and Museum, Nashville Zoo, Grand Ole Opry, and Madame Tussauds

  • 3 breakfasts

  • 1 dinner and cruise

  • 2 other dinners

  • Scholastica Travel Tour Leader

  • Motorcoach transportation: Holiday Tours

Cost:

  • Based on quad occupancy

    • 46-50 paid participants per bus = $924​

    • 41-45 paid participants per bus = $964

    • 36-40 paid participants per bus = $1014

    • 31-35 paid participants per bus = $1081

    • 26-30 paid participants per bus = $1172

Package Surcharges:

  • Quad occupancy = $0

  • Triple occupancy = $40 additional per person per package

  • Double occupancy = $120 additional per person per package

  • Single occupancy - $360 additional per person per package

Payment Schedule for Students and Chaperones (FORMS BELOW):

  • September 25 = $150 (Initial Permission Form and the CMS 5300 Form DUE for students and chaperones.)

  • October 25 = $150

  • November 25 = $150

  • December 18 = $100

  • January 17 = $150

  • February 19 = $150 (All other forms due.)

  • March 15 = $150 + $81 (Added this final payment to "Shop" page.)
    (This last amount may be more or less depending on our final numbers. We are between 31-35.)

  1. Initial Permission Form - due with Payment #1

  2. CMS Form 5300 - due with Payment #1

  3. BMS Band Medical Form - due with Payment #6

  4. CMS Medical Authorization Form - due with Payment #6 

  • for over-the-counter medication and prescription medication (Doctor's signature required!)

  • not needed if the student's parent is on the trip or student doesn't take any medications

   5. Scholastica Travel Medical Treatment Consent Form - due with Payment #6

   6. Behavior Contract - due with Payment #6

   7. Copy of Insurance Card - due with Payment #6

   8. Consent Form for Dinner Cruise 

   9. Travel Waiver (Only needed if students are to be released to a parent for the trip home.)
